We then created Risk Decomposition Diagram that shows the decisions that would need to be made at each phase and the associated consequences of these decisions in a more linear fashion. This diagram includes probabilities, which show that: 


Once a product fails a test more than once

The probability of deciding to continue investing in improving and re-testing it decreases because a company may find that it takes too much investment for the expected ROI.

 

Once a root cause analysis has been performed

The product has been redesigned in accordance with that root cause analysis, and retested, the probability that it passes the second test is higher than the probability that it fails that test.
